Ceiling fan switch?

I am having alot of trouble finding a replacement ceiling fan switch. I have been to appliance stores, hardware stores, etc and havent been able to find the switch I need. The switch has 5 wires on it, and there were 2 sets of numbers on each side. They were... 1/4 HP 125VAC 3A250VAC 0484M 174 95 403 If anyone can help me in finding this part, I would greatly appreciate it, unfortunatly I dont know the model or manufacturer of the ceiling fan. I wish I did because I am sure that it would help out alot, but all I know is what is on the switch itself, thanks.

1. You're going to have to find a lighting store that sells and services their lamps and ceiling fans. They might have a proper switch for you, but it's going to be tough to find. Although, unless it's a really expensive fan, I can't imagine why you don't just go buy a new one, they're not that expensive for all the trouble you're going to go through to fix this one.
2. Go to a Menards, or Lowes. You have a standard switch and they should be able to help.
